1. Medical Services (Non-Refundable)

All medical services, including consultations, evaluations, and provider time, are non-refundable once completed

2. Missed or Late Appointments

Missed appointments (“no-shows”) are non-refundable

Appointments canceled with less than [48 hours] notice may be subject to a cancellation fee or non-refund

3. Packages and Programs

Program fees (e.g., weight loss or HRT programs) are non-refundable once initiated

If a program has not yet started, refund requests may be considered on a case-by-case basis

Patient may cancel their membership at any time. To avoid renewal charges, cancellation requests must be submitted at least thirty (30) days prior to next billing date through secure portal or email.

No refunds will be issued for medications once prescribed or dispensed

This includes hormone therapy and weight loss medications due to safety and regulatory requirements

Lab fees are non-refundable once ordered or processed
